You may need to take further action by filling out a Change-Of-Address form, even if you have not moved recently or changed addresses. This is the only form available that you need to submit. Parents/Legal Guardians and Affiliates should complete the following steps:
- You will receive a Move Validation Letter at your old address. For your protection, this will not contain your new address. If you have already moved, no action is required.
- You should receive a mailed communication from the U.S. Postal Service at your new address within 7-10 business days of your mail start forwarding date.
- The person who submits this change of address form states that he or she is the person, executor, guardian, authorized officer, or agent of the person for whom mail would be forwarded under this order. Anyone submitting false or inaccurate information on this form is subject to punishment by fine or imprisonment or both under Sections 2, 1001, 1702 and 1708 of Title 18, United States Code.

Affiliates: If a parent continues to have an unresolved book delivery issue, there is an online option to assist a family in updating their address. Visit the USPS website and submit the Official Change of Address Form. When you click to print the form, check the box that you are assisting with an address change and it will ask for your contact information also.
